Yavapai County Community Health Services is offering several days of dedicated appointments for parents who need to get their child's required back to school shots. All vaccines required for school attendance will be available. Parents are reminded to be sure and bring the child's immunization record and, if applicable, the child's insurance card to the clinic. Appointments can be made by calling 928-583-1000 in the Prescott area and 928-639-8132 in the Verde Valley.
For children with current insurance coverage under AHCCCS or private insurance plans, those insurance plans will be billed directly. No child will be turned away due to lack of insurance coverage or inability to pay.
Immunization is one of the most important things a parent can do to protect their children's health. Immunizations are required for school attendance in the United States. Today we can protect children from 14 serious diseases.
Due to the vaccinations offered in the United States, smallpox was eradicated in 1977, and the U.S. was declared polio-free in 1994. The vaccine against Human Papillomavirus (HPV) is the first vaccine to prevent cancer (HPV is the leading cause of cervical cancer).

Prescott: 1090 Commerce Drive.
Wednesday, July 31 and August 7, from 1- 4 p.m.
Prescott Valley: 3212 n. Windsong Drive (second floor). Monday, July 29 from 8 a.m. -12 p.m. and 1-4 p.m., Wednesday, August 7, from 8 a.m.-12 p.m. and 1-4 p.m.
Cottonwood: 51 Brian Mickelsen Drive.
Monday, July 29 from 1-4 p.m.
Friday, August 2 from 8 a.m. to 12 p.m. and 1-4 p.m.
Friday, August 9 from 8 a.m. to 12 p.m. and 1-4 p.m.
